Medi-Cal and Assisted Living in San Diego
Medi-Cal enrollment does not automatically pay for a room in any assisted living community. Families need to identify the program, complete its assessment process and find a participating provider able to meet the person’s needs.
Reading about care is one thing; knowing which homes have a room this week and what they are actually charging is another. That part we find out for you, and it costs your family nothing.
Understand the Assisted Living Waiver
California’s Assisted Living Waiver, or ALW, supports eligible people who meet its nursing-facility-level care criteria and can receive services safely in an approved community setting. San Diego is a participating county. Review DHCS requirements rather than assuming Medi-Cal enrollment alone is enough. Official source
The waiver has specific eligibility rules, including full-scope Medi-Cal with zero share of cost. Ask a care coordination agency to explain current enrollment procedures and whether the applicant meets the program’s requirements.
Keep room and board in the budget
ALW pays for authorized services, not room and board. Participants remain responsible for housing payments and personal expenses, even when approved care services are funded. Official source
Ask for the residence’s written room-and-board arrangement and an explanation of any permitted additional expenses. Do not confuse state provider reimbursement rates with a resident’s cash benefit or housing allowance.
Use the care coordination and provider lists
Start with DHCS’s ALW information and care coordination resources. The assessment and enrollment process should establish whether this pathway is clinically and financially appropriate. Official source
Check the official participating-facility list, then confirm current participation, enrollment status, appropriate availability and care capability directly. A listing is not a promise of an open room. Official source
Plan for a gap between need and enrollment
Do not base an urgent hospital discharge on an unconfirmed waiver opening. Ask the discharge team and Medi-Cal plan about other appropriate supports while the waiver pathway is explored.
If remaining at home is feasible, ask San Diego County about IHSS eligibility and other local assistance. These are separate programs with their own assessments and limits, not interchangeable guarantees of residential placement. Official source
Questions families ask
- Does every assisted living residence accept Medi-Cal?
- No. Participation in a specific program must be verified, along with availability and the residence’s ability to meet assessed needs.
- Can the waiver pay the entire monthly housing bill?
- ALW does not cover room and board. Ask the care coordination agency and residence to explain the participant’s financial responsibilities before admission.
