Placement Handoffs That Support a Safer Discharge

A discharge destination is not ready because a room is vacant. A workable residential handoff requires a willing resident or authorized decision-maker, verified care capability, completed admission review, and a receiving team prepared for the first shift.

Reading about care is one thing; knowing which homes have a room this week and what they are actually charging is another. That part we find out for you, and it costs your family nothing.

Define the receiving level of care before searching

Describe the patient’s current function, not only the diagnosis or previous living arrangement. Transfer assistance, cognition, swallowing concerns, wound needs, behavior, and overnight monitoring can change whether an RCFE is appropriate.

California Health and Safety Code 1569.47 addresses placement-agency duties, including hospital discharge planners and coordinators, and prohibits placements where the required care is beyond the facility’s licensed limits. Do not substitute a referral vendor’s assurance for the receiving home’s assessment. Placement duties.

Send a focused, authorized referral packet

Confirm consent or other lawful authority for sharing information and use approved secure channels. Identify the resident’s preferences, decision-maker, available funding, desired area, and discharge target alongside the clinical summary.

Flag information that remains provisional. A tentative discharge date, pending equipment order, or unresolved medication change should be visible to the receiving administrator, not buried in a long record.

  • Current medical assessment and relevant clinical records.
  • Reconciled medications, prescriptions, allergies, and pharmacy plan.
  • Mobility, transfers, cognition, behavior, diet, and nighttime needs.
  • Equipment, outside services, follow-up appointments, and transport requirements.

Treat acceptance as a documented clinical and operational decision

The RCFE must complete its pre-admission appraisal and obtain the required medical assessment. A screening phone call or a photograph of an empty bedroom does not establish completed acceptance.

Confirm relevant fire clearance, condition-specific requirements, and any hospice or other approvals. Identify who provides each outside service and when it starts. If the patient needs skilled care, pursue an appropriate clinical setting rather than stretching residential capabilities. RCFE admission and care rules.

Plan the San Diego handoff around real travel and coverage

A discharge from UC San Diego Health’s Hillcrest or La Jolla locations, or from Sharp Chula Vista Medical Center, can place families on very different travel routes. Include the patient’s established follow-up clinics and the relatives who will participate, not just the closest available bed. UC San Diego Health locations. Sharp Chula Vista.

Before transport departs, confirm arrival time, the receiving staff contact, medications in hand or scheduled delivery, equipment installation, and escalation instructions. Check after-hours pharmacy and service coverage instead of assuming weekday arrangements extend through the weekend.

Keep choice, compensation, and unresolved barriers visible

If a commercial referral service participates, explain its role and ask how it is compensated. Do not present a participating network as every available option or imply a hospital endorsement that has not been authorized.

Document alternatives discussed, the patient’s choice, outstanding barriers, and the person responsible for each next step. Where discharge is disputed or appealed, follow the hospital’s applicable notice and review procedures; a referral search does not replace those protections.

Questions families ask

Can an RCFE accept someone directly from a hospital?
Potentially, if the home can legally and safely meet the person’s needs and completes the required admission process. Hospital discharge readiness alone does not establish RCFE suitability.
Does this guide promise a placement response time?
No. Availability, clinical review, documentation, approvals, funding, and patient choice determine the timeline. Confirm milestones with each receiving home.
What if no safe residential option is confirmed?
Escalate the specific barriers through the hospital’s discharge and clinical processes. Continue evaluating the appropriate level of care rather than treating an unverified vacancy as a completed plan.
