Family caregiving and IHSS

Getting Paid to Care for a Parent in San Diego

An adult child may be able to become a paid caregiver for an eligible parent through IHSS, but family caregiving does not automatically create a wage entitlement. The parent’s program approval and the caregiver’s enrollment are separate requirements.

Reading about care is one thing; knowing which homes have a room this week and what they are actually charging is another. That part we find out for you, and it costs your family nothing.

Establish the parent’s eligibility first

San Diego County IHSS helps eligible people remain safely at home with assistance such as personal care and household tasks. Eligibility includes Medi-Cal and financial requirements as well as a need for help. Official source

The assessment concerns what assistance the parent needs, not how much income the family caregiver has lost. Call 800-339-4661 to ask about applying and the information the county requires.

Complete the provider enrollment process

If the parent is approved and chooses an adult child as a provider, that caregiver must satisfy the applicable enrollment requirements before receiving program payment. CDSS directs prospective providers to complete the required enrollment steps and contact the county or IHSS Public Authority for assistance. Official source

Ask about orientation, forms, identity verification and background-check requirements. Do not assume that submitting the parent’s application also enrolls the adult child.

Understand what authorized hours mean

IHSS payment is tied to authorized services and hours under program rules. Being available all day does not mean every hour of companionship or family responsibility is payable.

Keep accurate records, follow timesheet requirements and ask the worker about tasks that are unclear. Confirm the effective dates and any rules about retroactive payment rather than promising yourself reimbursement for all past care.

Plan for needs the payment does not cover

A parent may need more supervision than the authorized hours provide. Build a backup plan for illness, appointments, respite and care that requires another worker or clinical professional.

The county’s Family Caregiver Support Program offers additional caregiver-focused resources, including respite and training. Availability and eligibility should be checked separately from IHSS. Official source

Questions families ask

Does Social Security pay an adult child to provide care?
Receiving Social Security does not by itself establish a paid family caregiver program. IHSS is a separate pathway that requires eligibility, assessment and provider enrollment.
Will IHSS cover all the time I spend with my parent?
Not necessarily. Payment depends on the services and hours authorized and the program’s rules, not simply on living together or being available.
