Organizing a skilled nursing admission

Nursing Home Placement in San Diego

Nursing home placement is an admission process involving clinical suitability, an available bed and a clear payment arrangement. It is different from selecting a nonmedical residential care home, even when both settings help with daily activities.

Reading about care is one thing; knowing which homes have a room this week and what they are actually charging is another. That part we find out for you, and it costs your family nothing.

Confirm why nursing-facility care is needed

Ask the treating team to identify the nursing, therapy or ongoing care needs that cannot be safely met in a less intensive setting. Clarify whether the goal is short-term rehabilitation or longer-term residence.

The answer shapes which facilities should review the referral and what funding questions to ask. A residential care provider and a skilled nursing facility should not be treated as interchangeable.

Coordinate the clinical referral

For a hospital transfer, the discharge planner typically helps send current medical information to potential facilities. Ask what is still needed for review, such as medication orders, therapy assessments or specialized treatment details.

Tell the team about dialysis, wounds, isolation needs, dementia symptoms or equipment requirements. A facility must confirm that it can meet the specific needs, not merely that it has a bed.

Verify the payment pathway

For a Medicare-covered skilled stay, ask whether eligibility requirements are met and what cost sharing or plan authorization applies. Medicare’s benefit is not unlimited long-term custodial coverage. Official source

For ongoing care, ask about Medi-Cal eligibility and whether the facility can accept the person under the proposed payment arrangement. Get a plan for pending applications and possible gaps rather than assuming a conversion will happen automatically.

Complete the agreement and handoff carefully

Review admission documents, responsible-party language, billing arrangements and the contact for care planning. Ask for clarification before signing anything that appears to create personal financial responsibility.

Confirm transport, medications, equipment and the receiving nurse’s handoff. Use CMS Care Compare to review candidate facilities, but remember that quality data does not confirm present-day availability or admission acceptance. Official source

Questions families ask

Does being referred by a hospital mean the nursing home must accept us?
No. The facility reviews care needs, capacity, payment arrangements and other applicable admission requirements before accepting a transfer.
Can a nursing home admit someone from home?
Sometimes, depending on clinical assessment, required orders, bed availability and payment. Admission from home does not by itself establish Medicare skilled nursing coverage.
