Comfort care in a familiar setting

Hospice in San Diego Care Homes

Hospice can sometimes come to a person living in a care home, allowing comfort-focused care without a hospital-based setting. The arrangement works only when the hospice team and the residence have clear, compatible responsibilities.

Reading about care is one thing; knowing which homes have a room this week and what they are actually charging is another. That part we find out for you, and it costs your family nothing.

Distinguish the residence from the hospice provider

The care home supplies housing and its agreed daily assistance. The hospice team provides the authorized hospice plan of care, with clinical services, equipment and medications related to the terminal illness as applicable.

Hospice eligibility is clinically determined. Medicare describes hospice as care for eligible people with a terminal illness, with care focused on comfort rather than curing the terminal condition. Official source

Confirm that the resident can remain there

Ask the administrator to review the person’s condition, required hospice permissions and the residence’s ability to provide daily care. Hospice involvement does not automatically authorize every residential setting to retain every resident.

Discuss increasing weakness, inability to transfer, swallowing changes and nighttime symptoms. Get a clear explanation of the conditions that might require a different setting.

Write down who responds and when

Ask who administers or assists with medications, who contacts hospice about pain and who stays with the resident between professional visits. Routine hospice does not mean a clinician is continuously at the bedside.

Create a shared contact plan for after-hours changes, equipment problems and urgent symptoms. Include the resident’s wishes and decision-maker so everyone knows how care decisions will be communicated.

Keep housing costs separate from hospice coverage

Medicare generally does not cover room and board where a person lives while receiving hospice. Certain short-term inpatient or respite stays arranged by the hospice team are treated differently under the benefit. Official source

Request separate explanations of residential charges and hospice-covered services. Families can compare Medicare-certified hospice providers through CMS Care Compare, while also assessing the residential home’s suitability. Official source

Questions families ask

Does hospice pay the care home’s monthly rent?
Generally not under Medicare’s hospice benefit. Room and board usually remains a separate responsibility, with limited exceptions for qualifying hospice-arranged inpatient or respite care.
Does choosing hospice mean there will be no medical support?
No. Hospice provides comfort-focused clinical support under a plan of care. Ask the team to explain symptom management, after-hours access and care for conditions unrelated to the terminal illness.
